Don't forget the AO settings in Render to File Settings>Options
Ambient Occlusion needs to be ON
"Occlusion sampling" determines the noise vs. speed trade off
It can be set very low for test renders and 30% is usually high enough for most final renders.
That works, it bring in the image and then it lets me select the image as a decal. Just doesn't seem to want to do it directly.
That should at least get you back to working on your project.
You should also be able to right-click on the Images folder and choose Import

what happens if you load an older PRJ that has some images in it already and try to add a new image to it?
During the trailers at the movies yesterday there was a commercial showing people doing absolutely amazing, mind-blowing, incredible things with Kinnect.

I think what you want is called a "Matte Object" in some circles.

This will need two passes and a compositing program that will allow you to work with alpha channels. Rendering to PNG may be usable but I prefer Targas since their alpha channel is more standard.
If you need this object to literally create an alpha channel hole like the camera background is... no, A:M doesn't have an object setting like that.
A second pass to create an image that could be used and composited with another alpha channel would be a likely solution.
An object set to be the same as the background color and set to 100% ambiance would appear the same as an object that showed the background color through it.
